Job Title

Schedule / Job Type

• Full-time, Nights, LTC/SNF setting, local only, rotating every other weekend off

Key Responsibilities

• Provide nursing care to residents in a long-term care or skilled nursing facility setting
• Administer medication and monitor patient health statuses
• Document patient assessments and care plans accurately
• Collaborate with interdisciplinary team to deliver quality resident care
• Ensure compliance with facility policies and healthcare regulations

Required Qualifications

• Valid LVN or LPN license in the state of West Virginia
• Minimum of one year of nursing experience in a LTC/SNF environment
• Ability to work 12.5-hour night shifts
• Strong communication and documentation skills
• CPR certification (preferred but not mandatory)

Preferred Experience / Skills

• Experience with electronic health records (EHR) systems
• Prior work in a similar skilled nursing or long-term care facility
• Ability to work in a rotating schedule with flexibility
