Job Title
• LVN / LPN - LTC/SNF

• Rio Rancho, NM

Schedule / Job Type
• Full-time, Nights shift, 12.5 hours per shift

Key Responsibilities
• Provide skilled nursing care in a long-term care and skilled nursing facility setting
• Maintain accurate documentation using PCC, MAR, and TAR systems
• Collaborate with healthcare team members, including Med Techs and other nurses, to ensure quality patient care
• Communicate effectively with patients and team members, ensuring patient needs are met and care plans are followed
• Demonstrate strong Point of Contact (POC) knowledge and professionalism while adhering to facility protocols

Required Qualifications
• Valid LVN or LPN license
• Experience with PCC, MAR, and TAR documentation systems
• Reliable and credentialed, with no history of no-call/no-show or frequent call outs
• Ability to work independently and as part of a team during night shifts

Preferred Experience / Skills
• Prior experience in LTC or SNF settings
• Strong communication skills and POC knowledge
