Key Responsibilities
- Provide wound care management and assessments for home health patients.
- Document patient progress and treatment plans using Homecare Homebase/OASIS within the last year.
- Conduct phone triage during on-call shifts, with minimal home visits.
- Collaborate with healthcare team to ensure quality patient care and timely interventions.
- Adhere to all safety and compliance guidelines, maintaining professionalism and compassion.
Required Qualifications
- Minimum of 2 years of home health wound care experience.
- Recent experience with Homecare Homebase/OASIS (within the last year).
- Proficiency with Wellsky EMR and wound care protocols a plus.
- Experience with on-call/triage duties.
- Valid professional licensure in hand at the time of submission.
- Reliable, dependable, and excellent time management skills.
Preferred Experience / Skills
- On-call/triage experience in home health settings.
- Familiarity with Wellsky EMR and wound care documentation.
- Strong customer service skills and compassionate approach.

Additional Details
- Work schedule: 5 days per week, 8.5-hour shifts (8:30 AM - 5:00 PM) with alternating weekends.
- On-call schedule: 1-2 weeks per month, 5:30 PM - 8:30 AM, primarily triage with minimal home visits.
- Pay: Guaranteed 40 hours per week.
- Contract length: 13 weeks.
