Key Responsibilities
- Provide comprehensive hospice nursing care to patients in a home health setting, focusing on end-of-life comfort and symptom management
- Administer medications utilizing advanced pain management protocols, including opioid titration and non-pharmacological interventions
- Conduct patient assessments, including symptom evaluation, mental status, and pressure ulcer staging, and document findings accurately
- Educate patients and families about end-of-life care, advanced directives, and coping strategies
- Collaborate with the care team to develop and implement individualized care plans aligned with Medicare and state regulations
- Respond to after-hours call rotations and manage patient needs efficiently during on-call periods
Required Qualifications
- Valid nursing license in North Carolina or a Compact state, with license in hand at start
- Minimum of 2 years of hospice or home health nursing experience
- Experience with medication administration, pain management, symptom assessment, and end-of-life care practices
- Basic Life Support (BLS) certification
- Ability to complete a 40-hour orientation and additional unit-specific training
Preferred Experience / Skills
- Epic electronic health record (EHR) experience or similar hospice EMR systems
- Additional certifications in palliative care, spiritual needs assessment, or legal/ethical issues in end-of-life care
- Experience with phlebotomy, IV management, and physician orders such as DME authorization
- Comfort with floating within the hospital or to nearby facilities as needed
