Job Title
• Registered Nurse - LTC/SNF

• Canistota, SD

Schedule / Job Type
• Full-time, 12-hour day shifts, every other weekend, holiday coverage as scheduled

Key Responsibilities
• Provide skilled nursing care to geriatric patients in a long-term care and skilled nursing facility
• Document patient conditions, treatments, and responses accurately using PCC EMR system
• Collaborate with nursing assistants and other healthcare team members to ensure quality patient care
• Manage elderly patients, including those with dementia, and handle behavioral health needs such as schizophrenia, bipolar disorder, and paranoia
• Conduct facility-specific orientation, including training, tours, competencies, and shadow shifts
• Adhere to facility policies and participate in shift-specific charting, including COVID-related documentation

Required Qualifications
• Valid state Registered Nurse (RN) license
• Experience in long-term care or skilled nursing facilities preferred
• Proficiency with electronic health records, specifically PCC system
• Ability to work 12-hour day shifts and handle geriatric patient care

Preferred Experience / Skills
• Geriatric psych experience is highly preferred
• Experience managing patients with dementia and mental health conditions such as schizophrenia and bipolar disorder
• Strong documentation and communication skills
