Schedule / Job Type
- Full-time, Monday through Friday, 8-hour shifts (8:00 AM - 4:30 PM)
- Weekends only during on-call shifts
- On-call duties once every 6 weeks, including potential holidays
Key Responsibilities
- Provide home health and hospice care for primarily geriatric patients with diagnoses such as heart disease, COPD, dementia, stroke, cancer, diabetes, wounds, and post-surgical care
- Conduct routine visits, OASIS assessments, and medication management, seeing approximately 5-6 patients daily
- Participate in daily huddle calls, weekly interdisciplinary team meetings, and monthly staff meetings
- Review utilization and ensure quality patient care during visits
- Educate patients and families on disease management and medication protocols
Required Qualifications
- Active RN license in the state of Wisconsin
- BLS certification required
- Experience with travel nursing, critical thinking, and chronic disease management
- Skills in wound care, IV administration, ostomy and central line care, and NPWT
- Hospice experience with pain management, symptom control, and end-of-life care
Preferred Experience / Skills
- Prior experience with Marshfield Health System (must have end date at least 6 months prior to submission)
- Experience with hospice and home health equipment such as CPAP or BiPAP (patients generally self-manage)

Additional Notes
- Certification in specific clinical skills preferred; no vents required
- Must be comfortable working in both home health and hospice settings, with no floating between departments

- This role involves comprehensive patient and family education, complex wound and medication management, and end-of-life care delivery
