Schedule / Job Type

• Full-time, days shift, 5 days per week, 8-hour shifts

Key Responsibilities

• Provide occupational therapy services to patients in a home health setting.
• Assess patients' needs and develop personalized treatment plans.
• Document patient progress using electronic charting systems such as EPIC and Oasis.
• Collaborate with multidisciplinary teams to ensure optimal patient care.
• Travel within the service area to see up to six patients per day, with minimal windshield time.
• Follow facility guidelines, including COVID-19 vaccination requirements and clearances.

Required Qualifications

• Minimum of 1 year of skilled home health experience as an occupational therapist.
• Valid OT license in the state of Oregon (pending accepted).
• BCLS certification.
• Reliable transportation for travel within the service area.
• COVID-19 vaccination, including booster, as per facility policy.

Preferred Experience / Skills

• Prior experience with Oasis and EPIC electronic health records.
• Comfortable working independently in a home health environment.
• Ability to manage a variable caseload within productivity expectations.
