Key Responsibilities

• Provide skilled nursing care to long-term acute care (LTAC) patients across various units, including subacute rehab, memory care, pulmonary care, and ventilator units.
• Float within hospital units as needed, delivering patient-centered care based on individual needs and care plans.
• Monitor and respond to patient alarms, especially on ventilator support units, demonstrating strong organizational and clinical skills.
• Follow strict attendance policies and report in accordance with facility protocols, including participation in mandatory orientation and training modules.
• Document patient care and coordinate with multidisciplinary teams to optimize patient outcomes.
• Adhere to infection control, safety procedures, and the facility’s policies to ensure a safe environment for patients and staff.

Required Qualifications

• Valid LVN or LPN license in Maryland or a Compact State.
• At least 1 year of experience in a long-term care or LTAC setting.
• Certifications in BLS and ACLS are required.
• Ability to work independently and handle complex patient needs, including ventilator management if applicable.
• Strong communication skills and attention to detail.

Preferred Experience / Skills

• Experience with subacute rehab, memory care, pulmonary, or ventilator care units.
• Familiarity with caring for patients with chronic conditions such as heart disease, diabetes, dementia, or those on ventilator support.
• Comfortable working in a fast-paced, high-complexity environment with strict adherence to attendance policies.
