Key Responsibilities

  • Supports and promotes the mission and values of the healthcare organization.
  • Performs therapeutic procedures under the supervision of a licensed therapist, following evaluation and treatment plans.
  • Implements therapeutic interventions aligned with residents' functional goals, including modalities and assistive device recommendations.
  • Communicates effectively with residents and caregivers, incorporating their input into care plans.
  • Monitors treatment progress, adapting plans as needed in collaboration with supervising therapists.
  • Documents all therapeutic sessions accurately and timely, ensuring accessibility for the care team.
  • Educates residents and caregivers on treatment goals and device usage, documenting educational outcomes.
  • Ensures compliance with regulations related to reimbursement programs such as Medicare and Medicaid.
  • Maintains a clean, safe, and organized clinical environment and follows infection control protocols.
  • Upholds required licenses, certifications, and competencies, completing annual compliance training.
  • Contributes to the development of new rehabilitation programs and services.
  • Participates in multidisciplinary team meetings and other departmental duties as assigned.

Required Qualifications

  • Associate degree in Physical Therapy Assistance or related field.
  • Current PT Assistant license.
  • Basic Life Support (BLS) certification.
  • Ability to lift up to 50 pounds; lifts over 50 pounds require two persons.

Preferred Experience / Skills

  • Prior experience in a long-term care or skilled nursing facility.
  • Strong communication and interpersonal skills.
  • Proficiency in Microsoft Office and documentation.
  • Compassionate, resident-centered care approach.
  • Good time management and decision-making skills.

Working Conditions / Physical Demands

  • Ability to perform physical tasks involving sitting, standing, walking, bending, and reaching.
  • Frequently lifts or carries up to 20 pounds; up to 50 pounds with assistance.
  • Capable of managing the physical and mental demands of the role with or without accommodations.
