Key Responsibilities
- Provide compassionate hospice care to patients in various settings, including private homes, nursing homes, group homes, and assisted living facilities
- Educate patients and families about care goals, symptom management, and treatment options
- Assess patient conditions and develop personalized care plans
- Lead and collaborate with the interdisciplinary team to ensure comprehensive patient care
- Offer emotional support and guidance to patients and their families during difficult times
- Maintain accurate documentation and ensure compliance with healthcare regulations
Required Qualifications
- Valid RN license in the state of Colorado
- Strong clinical skills in hospice and palliative care
- Ability to assess symptoms and develop effective treatment plans
- Excellent communication and teaching skills
- Critical thinking and assessment abilities
Preferred Experience / Skills
- Prior hospice or palliative care experience
- Experience with interdisciplinary team collaboration
- Knowledge of symptom management in various diseases
