Key Responsibilities
- Provide patient care in long-term care or skilled nursing facility settings
- Monitor patient health and record critical information accurately
- Collaborate with healthcare team to develop and implement care plans
- Ensure compliance with facility policies and healthcare regulations
- Assist with patient assessments and coordinate treatment as necessary
Required Qualifications
- Valid Registered Nurse (RN) license
- Experience in long-term care (LTC) or skilled nursing facilities (SNF) preferred
- Strong communication and documentation skills
- Ability to work independently and as part of a healthcare team
Preferred Experience / Skills
- Prior experience in a SNF or LTC setting
- Familiarity with electronic health records (EHR) systems
