Job Title

• Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) - LTC/SNF

Schedule / Job Type

• Full-time, night shifts, 3 shifts per week, 12-hour shifts, temp-to-perm

Key Responsibilities

• Assess and evaluate residents’ conditions
• Administer medications and treatments, including venipuncture blood draws
• Communicate condition changes to the interdisciplinary team
• Lead care coordination for residents with multiple comorbidities, including PTSD and memory care needs
• Manage and document patient care using PCC electronic medical records system
• Participate in team meetings and collaborate with RNs, physicians, and other healthcare professionals

Required Qualifications

• Active LPN or LVN license in the state of Montana
• Minimum of 1 year of experience in long-term care or skilled nursing facilities
• Experience with memory care and PTSD patient populations
• Proven competency in venipuncture blood draws
• Basic Life Support (BLS) certification

Preferred Experience / Skills

• Experience managing IV antibiotics and titratable drips (rare but accepted)
• Familiarity with telehealth management and managing respiratory treatments and equipment
• Ability to work flexibly within a team environment in a remote, rural setting
