LVN / LPN LTC/SNF

Job Title

• LVN / LPN (Licensed Vocational Nurse / Licensed Practical Nurse)

Schedule / Job Type

• Full-time, 12-hour night shifts, 3 shifts per week

Key Responsibilities

• Provide patient care to a long-term and rehab population in a skilled nursing facility
• Monitor patient conditions, administer medications, and document care in PCC electronic medical records
• Collaborate with healthcare team to ensure quality and consistency of care
• Adhere to facility policies regarding patient safety and care protocols
• Support patient transitions and recovery in a multidisciplinary environment

Required Qualifications

• Valid LVN or LPN license in the state of Minnesota
• Experience working in long-term care or skilled nursing facilities preferred
• Ability to accurately document in PCC or similar EMR systems
• Strong communication and interpersonal skills
• Flexibility to work three 12-hour night shifts per week

Preferred Experience / Skills

• Prior experience in a long-term care or rehabilitation setting
• Knowledge of common diagnoses and patient needs in a skilled nursing environment
• Ability to work independently and as part of a team
