Key Responsibilities

• Provide physical therapy assistant services to home health patients within designated territory.
• Travel to patient locations in Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont, Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, and Union City.
• Perform routine visits, documenting patient progress and updating records per agency and client standards.
• Adapt to patient needs, including Covid-positive and PUI cases, ensuring safety and compliance.
• Maintain licensed credentials and meet all required safety and health certifications.
• Work with the clinical team to deliver quality patient care and achieve visit goals, adhering to schedule and productivity requirements.

Required Qualifications

• Current California PTA License
• BLS certification
• Valid driver’s license, auto insurance, and registration
• TB test (annual), MMR, Varicella, Hep B, TDaP (within last 10 years), Flu (annual)
• Drug screening and physical exam clearance

Preferred Experience / Skills

• At least 1 year of professional experience as a Physical Therapy Assistant
• Home health experience preferred
• Willingness to travel outside designated area and work weekends if required
