Key Responsibilities

• Provide home health physical therapy services to patients in accordance with treatment plans.
• Ensure proficiency in documentation using OASIS for patient assessments.
• Document and communicate patient progress and treatment outcomes accurately.
• Comply with all travel and licensing requirements, including eligibility for local or travel assignments.
• Adhere to agency and facility policies, including charting and regulatory compliance.

Required Qualifications

• Valid state licensure as a Physical Therapist, or proof of compact licensure.
• Minimum of one year recent home health therapy experience.
• Proficiency in OASIS documentation standards.
• Current BLS, ACLS, and PALS certifications through the American Heart Association (AHA).
• CV or proof of appropriate licensure at submission.
• Must have or be able to obtain NIH and stroke-specific education (for acute care hospitals).

Preferred Experience / Skills

• Experience with EPIC electronic medical records system.
• Prior supervisory or leadership references.
• Recent specialty experience within the last year.
