Key Responsibilities
- Provide skilled nursing care to patients in a long-term care or skilled nursing facility setting.
- Monitor patient health status and report any changes to the medical team.
- Administer medication and treatments as prescribed.
- Document patient care activities accurately and thoroughly.
- Collaborate with interdisciplinary teams to ensure quality patient outcomes.
Required Qualifications
- Valid State Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license.
- Prior experience in LTC (Long-Term Care) or SNF (Skilled Nursing Facility) preferred.
- Ability to work 12.5-hour shifts.
- Strong interpersonal and communication skills.
- Reliable and dedicated to patient care.
Preferred Experience / Skills
- Experience working in a fast-paced healthcare environment.
- Knowledge of electronic medical records (EMR) systems.
