Key Responsibilities

• Provide outpatient physical therapy services to patients in a skilled nursing facility setting
• Develop and implement individualized treatment plans to improve patient mobility and overall function
• Document patient progress and communicate with healthcare team staff
• Ensure compliance with all relevant healthcare regulations and standards
• Contribute to patient recovery and quality of life improvement goals

Required Qualifications

• Valid state license as a Physical Therapist
• Professional experience working in a Skilled Nursing Facility (SNF) setting
• Strong knowledge of physical therapy practices and protocols
• Excellent communication and patient care skills

Preferred Experience / Skills

• Prior experience with outpatient therapy environments (preferred but not required)
• Ability to work effectively within a multidisciplinary team
