Job Title

Schedule / Job Type

• Full-time, Days shift, 5 shifts per week, 8-hour shifts

Key Responsibilities

• Provide compassionate end-of-life care to hospice patients in their homes or care facilities
• Assess patient needs and manage symptoms with clinical expertise
• Educate and support patients and their families regarding care goals and symptom management
• Collaborate with an interdisciplinary team to develop and implement personalized care plans
• Serve as a caregiver and teacher, empowering families with confidence in caregiving tasks
• Lead the care team for an assigned caseload of patients, ensuring quality and comfort

Required Qualifications

• Valid RN license in the state of Colorado
• Strong clinical assessment and critical thinking skills
• Experience in symptom management and palliative care preferred
• Ability to work independently and as part of a team
• Compassionate communication and patient education skills

Preferred Experience / Skills

• Experience in hospice, palliative, or home health nursing preferred
• Comfort in providing care across diverse settings including private homes, nursing homes, and assisted living facilities
