- Full-time, contract (8 weeks), dayshift, on-site
Key Responsibilities
- Provide discharge planning and care coordination for acute care patients
- Work across all hospital units, including possible float between departments
- Collaborate with multi-disciplinary teams to develop and implement care plans
- Document patient interactions and care plans in electronic health records (EPIC)
- Support patient transitions and ensure compliance with hospital protocols
Required Qualifications
- Active MSW or Social Worker license
- BLS certification
- Minimum of 1 year of acute care or hospital-based experience
- Strong discharge planning and care coordination skills
- Willingness to float across hospital units and campuses
Preferred Experience / Skills
- Experience with diverse case management including Med Surg, LLOS, and EDC
- Familiarity with EPIC charting system
- Previous hospital or trauma center experience

Additional Details
- Shift: 10-hour days, four days per week
- Start Date: ASAP
- Must work three major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, New Year’s Day)
- Candidates must have a Washington license or be eligible to obtain one
- Local candidates are not accepted; candidates must be outside a 50-mile radius and willing to float as needed
- References: Minimum of 2 supervisory references covering at least 6 months within the last 3 years in a comparable setting
- This position involves mandatory floating within hospital departments and possibly to sister hospitals within the area
