Key Responsibilities

• Provide rehabilitative therapy services to patients based on physician referrals and treatment plans.
• Assess patients' physical conditions and develop personalized therapy programs.
• Monitor and document patient progress and adjust treatment plans accordingly.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Educate patients and their families on therapy exercises and injury prevention strategies.
• Maintain compliance with all health and safety regulations and facility policies.

Required Qualifications

• Valid state license as a Physical Therapist.
• Proven experience in a clinical or outpatient setting.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a multidisciplinary team.
• Familiarity with relevant medical and therapy equipment.

Preferred Experience / Skills

• Experience working with diverse patient populations.
• Knowledge of relevant electronic medical record (EMR) systems.
