Key Responsibilities
- Provide physical therapy services to patients at a skilled facility
- Develop and implement individualized treatment plans
- Monitor patient progress and adjust treatments accordingly
- Collaborate with a multidisciplinary therapy team to ensure quality care
- Document patient evaluations and therapy sessions accurately
Required Qualifications
- Valid state license or certification as a Physical Therapist
- Proven experience in providing outpatient or inpatient therapy services
- Excellent communication and patient care skills
- Ability to work independently and as part of a team
Preferred Experience / Skills
- Experience working in a skilled nursing or rehab facility
- Familiarity with electronic health record systems
