Full-time, Days shift, 8-hour shifts, Monday through Friday with every 4th weekend rotation

Key Responsibilities

• Provide comprehensive home health nursing care to patients in accordance with physician orders and treatment plans.
• Document patient visits using EPIC electronic health records, focusing on point-of-care assessments and interventions.
• Adhere to all travel compliance requirements, including tiered travel policies and licensing regulations.
• Assist with patient education, chronic disease management, and medication administration within the home setting.
• Collaborate with healthcare team members to ensure quality patient outcomes and adherence to care plans.
• Complete required NIH and stroke education for applicable acute care hospital settings within Riverside Health facilities.

Required Qualifications

• Valid RN license in Virginia or compact license for multi-state practicing.
• At least 6 months of recent home health nursing experience.
• BLS certification through the American Heart Association (AHA).
• EPIC electronic charting experience.
• Must have current ACLS, PALS certifications through AHA.
• Must have a clean driving record and valid driver's license.
• Proof of immunizations and education related to NIH and stroke care for applicable facilities.
• SSN required at time of submission.

Preferred Experience / Skills

• Recent experience working in acute care hospital settings under Riverside Health.
• Experience with Medicare’s Outcome and Assessment Information Set (OASIS) documentation.
• Knowledge of the Riverside Healthcare system and hospital-specific protocols.
• Prior supervisory or team lead experience in home health settings.

Additional Notes
Candidates must select either Riverside Travel assignments or Riverside Per Diem contracts—they cannot do both simultaneously.
Travel and local candidates are accepted at the same base rate.
Candidates should be prepared for a possible floating assignment based on facility needs.
All certifications related to ACLS, PALS, and BLS must be current and through AHA.
RTO requests should be minimized and included during initial submission; no additional RTO requests will be approved after an offer is extended.
Prior employment as a travel RN must be inactive for at least 6 months before re-hiring.
One supervisory reference is required for submission; two are preferred for compliance.
