Key Responsibilities

• Provide direct social work services to patients and residents in a long-term care or senior living setting

• Assess patient and family needs and develop appropriate care plans in collaboration with the interdisciplinary team

• Facilitate discharge planning, community resource referrals, and transitions of care

• Maintain accurate and timely documentation in accordance with facility and regulatory standards

• Advocate for patient rights and support emotional and psychosocial well-being throughout the care continuum

Required Qualifications

• Active and applicable Minnesota Social Work (SW) license required

• Bachelor's degree in Social Work from an accredited program required

• Ability to commit to a full 8-week assignment with 40 guaranteed hours per week

• Must be available to work a Sunday through Saturday work week on day shifts

Preferred Experience / Skills

• Prior experience in a long-term care, skilled nursing, or senior living environment

• Familiarity with care planning, discharge planning, and interdisciplinary team collaboration

• Experience working with elderly or medically complex patient populations
