Schedule / Job Type
• Full-time, Days shifts, 1 shift per week

Key Responsibilities
• Collaborate with the multidisciplinary diabetes management team to provide comprehensive diabetes education to patients and families living with diabetes mellitus.
• Utilize the nursing process to develop, initiate, and execute individualized patient care plans in accordance with physician orders and clinic policies.
• Administer prescribed medications and treatments following approved nursing techniques, standards of care, and state laws.
• Document patient care activities accurately and in accordance with standards of practice.
• Support department performance improvement initiatives and participate in shared governance activities.
• Act as a resource for patient education program development and address patient education issues or challenges.
• Advocate for patients by addressing clinical, psychosocial, and financial concerns and escalating issues as needed.
• Float to various units based on clinical competency and identified needs, performing duties as assigned.

Required Qualifications
• Graduate of an accredited nursing program.
• Valid RN license in Illinois and Wisconsin.
• Certified in CPR (within 90 days of hire).
• CDCES certification required.

Preferred Experience / Skills
• Experience in a clinical setting providing patient education or diabetes management.
• Knowledge of holistic and culturally sensitive nursing care practices.
• Ability to work variable shifts, including weekends, holidays, and on-call hours.
