Key Responsibilities
- Provide physical therapy services to patients in VA and SNF settings.
- Develop and implement individualized treatment plans.
- Monitor patient progress and adjust treatments as necessary.
- Document patient care accurately and timely.
- Collaborate with healthcare teams to optimize patient outcomes.
Required Qualifications
- Valid state license to practice as a Physical Therapist.
- Experience in VA and SNF settings preferred.
- Strong knowledge of therapy techniques and patient care protocols.
- Excellent communication and documentation skills.
- Ability to work effectively in a team environment.
Preferred Experience / Skills
- Prior experience in a rehabilitation or skilled nursing facility.
- CPR and First Aid certification.
