Contract | 13-Week Assignment | Full-Time | Onsite | Days | Monday–Friday, 8-hour shifts | 40 hours per week

Key Responsibilities

• Provide direct patient care in a home health and hospice setting

• Participate in hospice team meetings on a biweekly basis

• Fulfill weekend rotation requirements approximately every 5 weekends, including scheduled visits and on-call coverage from Friday evening through Monday morning

• Respond to on-call duties 1–2 evenings per week from 5:00 PM to 8:00 AM, including emergency calls as needed

• Adhere to holiday coverage requirements based on unit needs

Required Qualifications

• Active Registered Nurse (RN) license

• Current Basic Life Support (BLS) certification

• Minimum of 1 year of hands-on nursing experience in acute care, home health, hospice, or inpatient settings

• Willingness to participate in on-call and weekend rotation schedules

• Ability to respond to and manage emergency calls during on-call hours

Preferred Experience / Skills

• Prior experience in home health or hospice care settings

• Demonstrated flexibility and willingness to coordinate and trade shifts with colleagues to meet patient care needs
