Key Responsibilities
- Perform comprehensive assessment, planning, intervention, and evaluation of inpatient and observation patients to ensure appropriate level of care and safe discharge planning, including complex pediatric cases with potential caseloads of ten or more patients per day
- Monitor length of stay (LOS) against GMLOS daily and manage cases per Milliman/Interqual Care Guidelines to reduce opportunity days and optimize LOS ratios
- Conduct utilization and resource management by assessing medical necessity, appropriateness of setting and services, and tracking variances using evidence-based criteria tools
- Collaborate proactively with social workers and the interdisciplinary team to formulate and execute discharge plans, initiate post-acute network referrals, and meet facility discharge time targets
- Utilize 3M reporting to assign and track working MS/APR DRGs daily; communicate and collaborate with nursing, physicians, and other healthcare professionals regarding LOS and documentation
- Refer cases to physician advisors for peer-to-peer reviews, medical necessity reviews, and denial avoidance; reconcile patient benefits with services rendered to minimize financial losses
Required Qualifications
- Current RN licensure in the State of Texas or valid compact state license required
- Graduate of an accredited School of Nursing required
- Minimum of 5 years of clinical practice experience for the population served
- Proficient word processing skills in a Windows environment
- Ability to analyze data and create reports and presentations using software applications
Preferred Experience / Skills
- BSN preferred; Certification in Case Management preferred
- Experience with Milliman (MCG) Guidelines and Interqual criteria highly preferred
- Familiarity with MIDAS and Meditech systems preferred; bilingual (English/Spanish) preferred
- Case management, resource management, and discharge planning experience preferred; CPR certification preferred
