Contract | Days | Monday–Friday, 12:00 PM – 8:30 PM | 13 Weeks
Key Responsibilities
- Provide hospital-based social work services within a behavioral health setting
- Conduct psychosocial assessments and develop individualized care plans for patients
- Coordinate discharge planning and connect patients with appropriate community resources
- Collaborate with interdisciplinary care teams to support patient treatment and transitions
- Maintain accurate and timely clinical documentation in accordance with facility standards
Required Qualifications
- Active Georgia Licensed Clinical Social Worker (LCSW) license required
- Minimum of 2 years of recent experience in social work specialty
- Acute hospital experience required
Preferred Experience / Skills
- Experience in a behavioral health hospital setting
- Familiarity with care coordination and community resource navigation
