Hospice / Outpatient Bereavement Coordinator

Key Responsibilities

• Provide bereavement support and counseling services to hospice patients and their family members throughout the final stages of illness and after loss
• Deliver medical social services and grief or loss counseling in home settings, including private residences, skilled nursing facilities, assisted living facilities, and adult foster homes
• Coordinate and support interdisciplinary hospice care teams serving patients across multiple counties
• Facilitate access to spiritual support, counseling, and trained hospice volunteer services as part of a comprehensive care model
• Maintain accurate documentation and participate in care planning consistent with hospice standards and regulations

Required Qualifications

• Bachelor's degree in social services, counseling, psychology, or a related field
• Experience or education in grief or loss counseling
• Current Healthcare Provider Level BLS certification
• Valid driver's license and personal auto liability/property damage insurance as required by law
• Must maintain personal vehicle in good working order, as travel to patient locations is required

Preferred Experience / Skills

• Master's degree in social services, counseling, psychology, or a related field
• Recent experience in a hospice setting
• Additional experience in a medical or healthcare environment
