LPN / LVN – Licensed Practical Nurse / Licensed Vocational Nurse

Contract | Nights | 12-hour shifts, 3-on / 3-off rotating schedule | Onsite

Key Responsibilities

• Assess and evaluate residents, monitor and communicate condition changes to the interdisciplinary team
• Administer medications and treatments, including IV antibiotic drips as needed
• Perform venipuncture blood draws
• Coordinate and lead resident care planning in collaboration with PT, OT, SLP, social workers, and dieticians
• Manage telehealth visits and utilize electronic medical order and communication systems
• Provide care across a long-term care setting including a locked memory care unit, serving residents with multiple comorbidities including PTSD

Required Qualifications

• Active LPN or LVN license in good standing
• Minimum 1 year of LTC / Skilled Nursing Facility experience
• Experience with memory care and/or PTSD patient populations
• Proficiency in venipuncture blood draws
• Current BLS certification

Preferred Experience / Skills

• Experience with PCC (PointClickCare) electronic medical records
• Familiarity with telehealth management and respiratory equipment oversight
• Willingness to pick up additional shifts and flexibility to support permanent staff scheduling gaps
