Contract | Days | Full-Time | 5x8-Hour Shifts (8:00–16:30) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to home health patients in Seaside, OR and surrounding areas
• Conduct patient evaluations and deliver individualized treatment plans in a home-based setting

• Complete Oasis assessments accurately and in a timely manner
• Document patient care using EPIC Remote Client for Home Health and the Rover mobile application
• Manage a caseload of up to 6 patient visits per day while meeting standard productivity expectations
• Travel to patient homes within the service area, with an occasional maximum daily drive radius of approximately 60 miles

Required Qualifications

• Minimum of 1 year of skilled home health physical therapy experience
• Active or pending Physical Therapist license in Oregon
• Basic Cardiac Life Support (BCLS) certification required
• Proficiency with Oasis documentation
• Reliable personal transportation required
• COVID-19 primary vaccination series plus one booster required

Preferred Experience / Skills

• Experience with EPIC Remote Client or similar home health electronic medical record systems
• Familiarity with productivity point-based caseload management in a home health environment
• Comfort navigating rural or geographically dispersed patient territories
