Job Title

• Physical Therapist (SNF & AL/IL)

Schedule / Job Type

• Full-time, Days shifts, 8 hours per shift

Key Responsibilities

• Provide skilled physical therapy services to residents in skilled nursing and assisted living/independent living facilities
• Develop and implement individualized treatment plans to improve patient mobility and function
• Collaborate with healthcare team members to ensure optimal patient care
• Document patient progress and maintain accurate medical records
• Assist in patient assessments and modify treatment approaches as needed

Required Qualifications

• Valid state license to practice as a Physical Therapist Assistant
• Proven experience in SNF and AL/IL settings preferred
• Excellent interpersonal and communication skills
• Ability to work independently and as part of a team
• Commitment to patient care and ethical practices

Preferred Experience / Skills

• Experience working with diverse patient populations
• Knowledge of state and federal healthcare regulations
• CPR/BLS certification preferred
