Contract | Days | 3x8-Hour Shifts, Wednesday–Friday, 8:30 AM – 5:00 PM
Key Responsibilities
- Provide skilled speech-language pathology services to home health patients in a 1:1 care setting
- Visit up to 6 patients per day within an assigned geographic service area, with occasional travel radius of 50+ miles
- Complete Oasis assessments and maintain accurate clinical documentation using EPIC
- Meet standard productivity expectations of 30 productivity points per shift
- Coordinate with the care team to group patient visits efficiently and minimize drive time
Required Qualifications
- Minimum 1 year of skilled home health SLP experience
- Active or pending Speech Language Pathologist license (Oregon)
- Basic Cardiac Life Support (BCLS) certification required
- Oasis experience required
- Reliable personal transportation required
- COVID-19 primary vaccination series plus one booster required
Preferred Experience / Skills
- Experience with EPIC electronic health records
- Familiarity with home health productivity standards and point-based expectations
