Key Responsibilities
- Provide skilled physical therapy services to patients in a skilled nursing facility (SNF) setting.
- Develop and implement individualized treatment plans to improve patient mobility and function.
- Document patient progress and communicate with interdisciplinary team members.
- Collaborate with staff to ensure quality care and compliance with healthcare regulations.
Required Qualifications
- Minimum of 2 years of professional SNF experience.
- Valid state license to practice as a Physical Therapist.
- Proven ability to work independently and as part of a team.
- Strong clinical and documentation skills.
Preferred Experience / Skills
- Experience working with elderly populations in skilled nursing or rehabilitation centers.
- Knowledge of healthcare regulations and compliance standards.
