Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Evaluate, assess, and develop individualized treatment plans based on patient needs
• Deliver therapeutic interventions and monitor patient progress throughout the course of care
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with the care team to ensure continuity and quality of patient care

Required Qualifications

• Active Physical Therapist license required (permanent state license required; compact license alone is not sufficient)
• Prior experience in an outpatient physical therapy setting
• Ability to meet credentialing and contract quality assurance requirements prior to start
• Must not have worked at this facility in a permanent or PRN capacity within the past year

Preferred Experience / Skills

• Experience working in a high-volume outpatient rehabilitation environment
• Familiarity with travel or contract therapy assignments
