LVN / LPN – Hospital Float (LTAC)

Contract | Nights (3x12 Hours, 7:00 PM – 7:30 AM) | Onsite | Every Other Weekend Required

Key Responsibilities

• Float across multiple units including pulmonary care, ventilator care, subacute rehabilitation, long-term care, and memory care units
• Assess patient status and implement clinical nursing interventions to modify symptoms and improve outcomes
• Develop and implement individualized patient care plans based on each resident's needs, routines, likes, and dislikes
• Provide care for patients with complex and chronic conditions including post-operative, post-stroke, post-cardiac, wound care, dementia, and chronic respiratory needs including tracheostomy and ventilator-dependent patients
• Respond appropriately to ventilator alarms and apply specialized training for vent patient care
• Utilize strong organizational and leadership skills in a fast-paced, high-acuity environment

Required Qualifications

• Valid LPN / LVN license in Maryland or a Compact state (required at time of submission)
• Minimum of one (1) year of LPN / LVN experience
• Current ACLS and BLS certifications required
• Ability to float within the facility across multiple units and within scope of practice
• Must be available to work every other holiday as required (New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas)
• Must be able to attend all required orientation days as scheduled

Preferred Experience / Skills

• Experience with ventilator-dependent patients and tracheostomy care
• Background in subacute rehabilitation, long-term care, or memory care settings
• Experience with wound care assessment and prevention
