LVN / LPN – Hospital Float (LTAC)

Contract | Nights (3x12 Hours, 7:00 PM – 7:30 AM) | Onsite | Every Other Weekend Required
Key Responsibilities
- Float across multiple units including pulmonary care, ventilator care, subacute rehabilitation, long-term care, and memory care units
- Assess patient status and implement clinical nursing interventions to modify symptoms and improve outcomes
- Develop and implement individualized patient care plans based on each resident's needs, routines, likes, and dislikes
- Provide care for patients with complex and chronic conditions including post-operative, post-stroke, post-cardiac, wound care, dementia, and chronic respiratory needs including tracheostomy and ventilator-dependent patients
- Respond appropriately to ventilator alarms and apply specialized training for vent patient care
- Utilize strong organizational and leadership skills in a fast-paced, high-acuity environment
Required Qualifications
- Valid LPN / LVN license in Maryland or a Compact state (required at time of submission)
- Minimum of one (1) year of LPN / LVN experience
- Current ACLS and BLS certifications required
- Ability to float within the facility across multiple units and within scope of practice
- Must be available to work every other holiday as required (New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas)
- Must be able to attend all required orientation days as scheduled
Preferred Experience / Skills
- Experience with ventilator-dependent patients and tracheostomy care
- Background in subacute rehabilitation, long-term care, or memory care settings
- Experience with wound care assessment and prevention
