Hospice Registered Nurse – Care Manager

Schedule / Job Type

Contract | Days, Monday through Friday with some weekend coverage | Onsite / Field-Based

Key Responsibilities

• Provide compassionate end-of-life care to hospice patients in their home setting, addressing both physical and emotional needs
• Deliver expert pain and symptom management to ensure patient comfort and quality of life
• Collaborate with an interdisciplinary team, physicians, and community providers to coordinate and optimize patient care
• Perform OASIS assessments and manage documentation in accordance with home health and hospice standards
• Serve as a case manager for an assigned patient caseload, coordinating services and care planning

Required Qualifications

• Active Registered Nurse (RN) license in good standing
• OASIS experience is required
• Home health experience is required
• Experience in hospice or end-of-life care
• Ability to perform case management duties in a home-based care environment

Preferred Experience / Skills

• Prior experience as a home care traveler
• Familiarity with interdisciplinary care team collaboration in a hospice setting
