Key Responsibilities
- Provide compassionate end-of-life care, addressing the physical and emotional well-being of patients and their families in a home setting
- Deliver expert pain and symptom management to ensure patient comfort during end-of-life care
- Collaborate with an interdisciplinary team, physicians, and community providers to optimize patient quality of life
- Perform OASIS assessments and contribute to case management activities
Required Qualifications
- Active Registered Nurse (RN) license in good standing
- OASIS experience required
- Home health experience required
- Ability to independently manage a patient caseload in a home care environment
Preferred Experience / Skills
- Prior experience as a home care traveler
- Experience in hospice or end-of-life care settings
- Familiarity with interdisciplinary care team collaboration
