Key Responsibilities
- Provide individualized, goal-oriented nursing care to home health patients using the nursing process in accordance with physician orders
- Serve as Case Manager for cases in which the RN is the lead discipline
- Deliver home health services to patients of varying backgrounds and needs across the community
- Support holistic care approaches including health promotion and long-term care services
Required Qualifications
- Active Registered Nurse (RN) license in the state of Vermont
- Experience in home health nursing
- Ability to apply the nursing process to develop and implement individualized patient care plans
- Strong communication and organizational skills to manage a patient caseload
Preferred Experience / Skills
- Prior experience in a case management role within a home health setting
- Familiarity with hospice or maternal-and-child health services
