Contract | Days | Full-Time | 5x8-Hour Shifts (8:00–16:30) | Onsite / Field-Based
Key Responsibilities
- Provide skilled home health nursing care to patients in the West Portland area and surrounding communities
- Conduct patient visits in the field, with up to 6 patients per day and a standard productivity expectation of 30 points per day
- Complete clinical documentation using EPIC Remote Client and the Rover mobile application
- Perform and complete OASIS assessments accurately and in a timely manner
- Manage daily travel to patient locations, with occasional maximum drive radius of approximately 60 miles per day
Required Qualifications
- Active Oregon Registered Nurse (RN) license
- Minimum 1 year of skilled home health nursing experience
- Current Basic Cardiac Life Support (BCLS) certification
- Demonstrated OASIS experience (must be disclosed at time of submission if not applicable)
- Reliable personal transportation required
- COVID-19 primary vaccination series plus one booster required
Preferred Experience / Skills
- Familiarity with EPIC Remote Client for home health charting
- Experience meeting productivity point expectations in a home health setting
- Comfort navigating a broad geographic service area with efficient time management
