Key Responsibilities
- Conduct comprehensive assessments of patient and caregiver needs using the nursing process, and develop individualized, realistic plans of care in accordance with state and federal regulations.
- Serve as Case Manager for assigned patients, actively managing caseloads and visit schedules in alignment with the interdisciplinary team (IDT) plan of care.
- Participate in IDT meetings to develop and review individualized care plans within five days of admission and every fifteen days thereafter.
- Implement medical plans of care, nursing interventions, and procedures while maintaining current knowledge of pain control, symptom management, IV use, wound VAC, and complex wound care.
- Perform patient and caregiver education on topics including medication side effects, treatment options, discharge instructions, and available services.
- Coordinate patient care with physicians, providers, team members, and vendors; facilitate safe discharges, transfers, and accurate completion of required agency, state, and federal documentation.
Required Qualifications
- Active Registered Nurse (RN) licensure in the state of Vermont or eligibility for licensure.
- Knowledge of state and federal regulations governing hospice and home health nursing practice.
- Proficiency in nursing documentation and electronic health record systems.
- Ability to delegate appropriately to LPNs within their scope of practice and oversee plan of care compliance.
- Strong skills in pain management, symptom control, and high-tech clinical procedures including IV therapy and complex wound care.
Preferred Experience / Skills
- Prior experience in hospice, palliative care, or home health nursing.
- Familiarity with Medicare and insurance authorization processes, including communication with insurance case managers.
- Experience conducting supervisory visits and participating in peer review audits.
