Key Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans
- Provide direct physical therapy services in a skilled nursing and rehabilitation setting
- Document patient progress and adjust treatment plans as appropriate
- Collaborate with interdisciplinary care team to support patient recovery goals
Required Qualifications
- Active Physical Therapist (PT) license in the state of Texas
- Graduate of an accredited Physical Therapy program
- Experience working in a skilled nursing or rehabilitation environment
Preferred Experience / Skills
- Prior experience in a long-term care or post-acute rehabilitation setting
- Familiarity with Medicare documentation and compliance requirements
