Key Responsibilities

• Provide intermittent skilled nursing visits to hospice patients in home settings, including private residences, skilled nursing facilities, assisted living facilities, and adult foster homes
• Deliver pain and symptom management services to patients in the final stages of illness
• Participate in 24-hour on-call nursing coverage as part of a rotating hospice care team
• Coordinate and collaborate with an interdisciplinary team including medical social workers, counselors, spiritual support staff, hospice aides, and volunteers
• Support patients and family members throughout the end-of-life care process, including bereavement support coordination

Required Qualifications

• Current, unencumbered Oregon RN license required upon submission
• Healthcare Provider Level BLS certification required
• Valid driver's license and personal auto liability/property damage insurance as required by law
• Must maintain personal vehicle in good working order, as travel to patient locations is required

Preferred Experience / Skills

• BSN degree and/or previous home health, hospice, or case management experience
• Broad-based nursing background with specialized skills applicable to home health and hospice nursing
