Full-time, Day shifts, 5 days per week

Key Responsibilities

• Provide skilled home health nursing services to patients in their residences
• Manage a patient caseload with a 1:1 ratio, visiting up to 6 patients per day
• Document patient care in electronic health record systems such as EPIC and Oasis
• Collaborate with healthcare team members to develop and implement care plans
• Ensure compliance with all client and regulatory requirements, including COVID vaccination policies

Required Qualifications

• Active Oregon or Washington RN licensure (pending licenses accepted)
• Minimum of 1 year of skilled home health nursing experience
• BLS certification
• Experience with Oasis documentation required
• Own reliable transportation for patient visits

Preferred Experience / Skills

• Previous experience in hospital or similar healthcare settings
• Familiarity with home health software applications such as Oasis, EPIC, and Rover
• Ability to work independently and efficiently
