Contract | Days | Full-Time | 5x8-Hour Shifts (8:00–16:30) | Onsite / Field-Based
Key Responsibilities
- Provide skilled nursing care to home health patients in the East Portland area and surrounding communities
- Visit up to 6 patients per day, with scheduling designed to minimize drive and windshield time
- Complete clinical documentation using EPIC Remote Client and Rover mobile application
- Perform and document OASIS assessments accurately and in a timely manner
- Travel independently to patient homes within the service area, with an occasional maximum daily radius of approximately 60 miles
Required Qualifications
- Active Registered Nurse licensure in both Oregon and Washington (pending licensure considered)
- Minimum of 1 year of skilled home health nursing experience
- Basic Cardiac Life Support (BCLS) certification required
- Proficiency with OASIS documentation
- Valid driver's license and reliable personal transportation
- Primary COVID-19 vaccination series plus one booster required
Preferred Experience / Skills
- Experience with EPIC Remote Client for home health charting
- Familiarity with the Rover mobile application for clinical documentation
- Comfort working independently in a field-based, patient-facing environment
