Physical Therapist – Home Health
Location
Missoula, MT (with coverage in surrounding counties)
Schedule / Job Type
Contract | Full-Time | Days | Monday through Friday, 8am–5pm | Rotating on-call, weekend, and holiday coverage included
Key Responsibilities
- Evaluate patients and establish individualized plans of care to meet needs within the home environment, coordinating with other disciplines and obtaining orders for additional services as appropriate
- Provide direct patient care in the home setting based on established care plans and physician orders
- Re-evaluate patient status and progress, modifying treatment plans including frequency and duration of care as needed
- Communicate effectively with physicians, scheduling staff, and other team members, and document all skilled services provided
- Oversee services provided by Physical Therapist Assistants when applicable
- Participate in rotating on-call, weekend, and holiday coverage
Required Qualifications
- Bachelor of Science degree or higher from an accredited Physical Therapy program
- Current Physical Therapist license to practice in the State of Montana
- Valid driver's license and appropriate auto insurance
Preferred Experience / Skills
- Minimum of two years of Physical Therapy experience
- Prior experience in a home health setting
