Contract | Full-Time | Onsite | Day Shift (5x8 Hours, Monday–Friday, 8:00 AM–5:00 PM)
Key Responsibilities
- Provide physical therapy services to adult patients in a home health setting
- Manage a caseload of approximately 6 patient visits per day
- Cover assigned service areas across multiple counties
- Document patient care and progress accurately and in a timely manner
- Complete required orientation modules and annual training through the designated learning portal
Required Qualifications
- Valid Maryland Physical Therapist state license required at time of submission
- Minimum of 1 year of physical therapy experience
- Home health experience required
- Current BLS certification required
- Experience working with adult patient populations required
Preferred Experience / Skills
- Experience with HCHB (HomeCare HomeBase) documentation software preferred
- Ability to manage a flexible schedule across multiple county service areas
