Schedule / Job Type

• Full-time, Night shifts, 8-hour shifts, 5 days per week

Key Responsibilities

• Provide nursing care to patients with diagnoses such as dementia, diabetes, hypertension, chronic kidney disease, and post-fall assessments.
• Assist with activities of daily living (ADL) support and patient safety.
• Document patient care accurately using PCC electronic medical record system.
• Follow facility-specific orientation and protocols.
• Adhere to staffing ratios, typically 1:22, ensuring quality patient care.
• Comply with facility policies regarding shift attendance and reporting.

Required Qualifications

• Valid state Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license.
• IV Care Certification (IV-C) required for LPNs.
• Experience with dementia, chronic illness, and elderly patient care preferred.
• Ability to work nights and flexible shifts, including weekends and holidays.

Preferred Experience / Skills

• Previous experience in long-term care or skilled nursing facilities.
• Familiarity with PCC Electronic Medical Records.
• Strong communication and patient care skills.
