Contract | Days | Full-Time | Onsite | Monday through Sunday rotation; 10-hour day shifts with weekend rotation as required
Key Responsibilities
- Provide end-of-life care including complex symptom assessment, management, and patient/caregiver education across home, SNF/ALF, and inpatient hospice settings
- Initiate and participate in goals-of-care discussions and care planning with patients, families, and the interdisciplinary team
- Manage and administer medications via multiple routes including IV, subcutaneous, oral, sublingual, and rectal; maintain central lines including PORT/IVAD, PICC, and other central lines
- Perform unit-specific procedures including pain management, continuous infusion initiation, palliative sedation, tracheostomy care, wound care, ostomy and drain management, and venipuncture
- Operate and monitor specialized equipment including CADD pumps, IV pumps, BiPap, CPAP, cough assist machines, and nebulizers
- Document clinical findings and care plans accurately using a narrative-heavy electronic medical record system; manage an assigned geographic patient caseload
Required Qualifications
- Active Registered Nurse (RN) license required at time of submission
- Minimum of 3 years of community hospice nursing experience
- American Heart Association (AHA) Basic Life Support (BLS) certification required
- EPIC EMR experience within the past 2 years — required and non-negotiable
- Strong critical thinking skills with demonstrated ability to perform comprehensive end-of-life assessments and symptom management
- Proficient typing skills required due to narrative-heavy documentation system
Preferred Experience / Skills
- 3–5 years of overall clinical nursing experience
- Experience floating between home hospice and inpatient hospice settings
- Familiarity with Net Smart or similar hospice-focused EMR platforms
- Experience caring for both adult and pediatric hospice patient populations
