LVN / LPN LTC/SNF

Job Title

• Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

• Santa Fe, NM

Schedule / Job Type

• Part-time, Night shifts, 3 shifts per week

Key Responsibilities

• Provide basic nursing care to residents in long-term care or skilled nursing facilities (LTC/SNF) setting.
• Monitor and record patient health status, including vital signs and observations.
• Assist with patient mobility, hygiene, and other daily activities.
• Follow physician's orders and maintain detailed patient documentation.
• Collaborate with healthcare team to ensure quality care delivery.

Required Qualifications

• Valid LVN or LPN license in the state of New Mexico.
• Ability to work night shifts with 12.5-hour shifts.
• Prior experience in LTC or SNF environments preferred.
• Strong communication and patient care skills.
• Ability to stand for extended periods and lift patients as needed.

Preferred Experience / Skills

• Previous experience working nights in a long-term care or skilled nursing facility.
• Knowledge of electronic health records (EHR) systems.
