Contract | Days | 5x8 Hours (8:00 AM – 4:30 PM)
Key Responsibilities
- Provide physical therapy services to patients in a home health setting
- Complete and maintain accurate OASIS documentation in accordance with regulatory requirements
- Develop and implement individualized patient care plans based on assessment findings
- Collaborate with interdisciplinary care team to coordinate patient treatment and progress
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
Required Qualifications
- Active Physical Therapist license valid in Virginia (in-hand at time of submittal or proof of compact license)
- Minimum 1 year of experience in home health physical therapy
- Proficiency in OASIS documentation
- Current BLS certification through the American Heart Association (AHA)
- One supervisory reference required at time of submittal
Preferred Experience / Skills
- Experience with home health-specific electronic medical record systems
- Strong organizational and time management skills suited for independent home health practice
