Key Responsibilities

• Provide home health physical therapy services to patients in the Baytown and surrounding areas
• Complete the required weekly visit point quota (32–34 points per week) based on a 1-point-per-visit structure
• Contact the scheduling team for all missed visits and changes in visit frequency or patient contact issues
• Document all patient care and clinical notes using Wellsky charting software
• Adhere to professional dress standards including clean scrubs and closed-toe shoes
• Report drive time accurately; overtime is compensated when drive time causes hours to exceed 40 per week

Required Qualifications

• Active Physical Therapist (PT) license in the state of Texas
• Minimum of 2 years of recent licensed PT experience (private duty experience does not qualify; new graduates will not be considered)
• Home health experience required
• Current BLS and First Aid certification
• Proficiency or ability to learn Wellsky electronic charting system

Preferred Experience / Skills

• Prior travel therapy assignment experience is a plus, though this may serve as a first travel assignment for qualified candidates with home health background
• Familiarity with GSA mileage reimbursement processes and paid drive time structures
